Failure Modes and Effect Analysis
A huge column table with numbered calculations that will help you list out the possible failures and the risks associated with a potential failure in the process to the customer.
Collecting Data
Collecting data- it is needed in this phase to collect data and create baseline measurements. Hopefully there are already calculated metrics or there needs to be process monitoring.
Continous Versus Discrete Data
Discrete Data: categorical (ordinal, nominal, binary, and attribute data)
Continuous Data: quantitative data with rank and infinite values (hrs per day, temp in degrees, length)
Levels of Data (lowest-highest)
nominal data- applies number labels to a qualitative description.
ordinal data- the data is nominal but has ranking associated with it (pain severity 1-10)
interval data- there is meaning associated with the changes in the steps of the data, but no ranking.
ratio data- has the highest ranking where there is an absolute zero point.
Choosing the Best Measurement Systems
goal of a measurement system is accuracy.
-a collector should have a booklet on how to accurately collect the data.
-use 10-bucket rule for accuracy
Gage R&R
Gaging Repeatability & Reproducibility:
repeatability- can one sample tester come up with the same measurements over and over again?
reproducibility- can multiple sample testers come up with the same measurements over and over again?
Collecting Data
Data Samples are collected as such:
- Simple Random Sampling
- Stratified Sampling
- Sequential Sampling
Measure Tollgate Checklist
Things to do before moving from “Measure Phase:”
1.) Team has agreed on a measurement + created a baseline measurement
- ) analyzed there chosen measurement and tested for reproducibility +repeatability
- )The team has removed error risks from measurement systems.
- ) The team has calculated the Sigma levels + variation levels
- ) sampling has been done in a respectable way to create baseline measurements
- ) the sponsor/champion has signed off on the measurement phase.
