what are goals of care?
are designations used by your healthcare providers to describe the general aims of your healthcare and the preferred location of that care
what is an advanced care plan?
a verbal or written summary of an adult’s wishes or instructions about the kind of care they want or do not want if they cannot express themselves
what is the purpose of ACP?
- ensure clear understanding of illness, its trajectory, and available treatment options
- prioritizes the expression and understanding of the beliefs and values of the patient and how those will influence healthcare related preferences and decisions
define personal directive
a legal document that names your agent or substitute decision maker; personal directive comes into effect if you lose capacity to make decisions for yourself
define capacity
the ability to make an informed choice; capacity may change over time or based on the situation
define agent/substitute decision maker
an individual chosen to make decisions on your behalf in the event that you are unable to make those decisions yourself
define power of attorney
is a legal document that you make to give another person the authority to make financial decisions on your behalf
