Glomerular Diseases and Conditions Flashcards
What are the two Glomerular conditions
Glomerulosclerosis/Nephrosclerosis- Scaring
Glomerulonphritis- Inflamation
Nephrotic Syndrome
Glomerulosclerosis/Nephrosclerosis is
Scarring of the glomeruli or Sclerosis- In several conditions of sclerotic such as lupus or diabetes this can occur. it is caused by the activiation of the glomerulal cells produceing scar tissues.
DIABETIC NEPHROPATY is a major cause of this.
Acute Glomerulonephritis is
the result of infections. It includes-
Strep infections (impentigo{rarely})
HIV-5-10 % develop kidney failure before fullblown aids
Bacterial endocarditis(infection of the heart)
T
Symptoms of Glomerular disorders include
- hematuria
- edema (facial/periorbital)
- Azotemia-accumlation of nitrogenous wastes in blood(BUN)
- urine cola colored
- hypertension
- hypoalbuminemia
- ^ BUN & Creatinine
- Headache
- Protienurea, oliguria, dysuria
Managment of issues of Glomerular disorders
- Treat S/S such as ^ BP
- Check GFR with 24 hour Creatinine clearance
- Treat Strep infections with antibiotics(PCN penicillin)
- Coriticosteroids(reduce inflamation)
- Limit dietary protiens and increase carbs
- restrict sodium
- If progresses to Chronic condition may be treated as as CKD
Complications of Glomerurlar conditions
- Hypertensive Encephalopathy
- Heart Failure(increase in Arthmyias
- Rapid decline in Renal Function can occur to ERSD
Nephrotic Syndrome
This is not a specific disease but a set of symptoms that can occur with any disorder of the kidneys,
Nephrotic syndrome is:
- a cluster of findings
- a condition of increased Glomerular permeability
- Results in Massive Protien Loss
- Marked increased in protien in urine from loss
- from albumin in urine
- Foamy urine from protein
- Hypoalbuminemia- low albumin
- Edema(full body) know as Anasarca
- High Serum cholesterol and LDL
- Increased weight from build up of fluids
*
1 liter of fluid is equal to _________ Kilograms or ______lbs
1 Kilogram or 2.2 Lbs
Treatment of Nephrotic syndrome includes
- Renal biopsy to determine cause
- Steroids
- immunosupressive agents
- ACE Inhibitors (prils)
- Loop Diuretics
- Statins(which is controversial)
- LIMIT SODIUM INTAKE
