when getting a health history for an GI assessment what questions should be asked?
- diet, area of pain (PQRST)
- physical activity
- nausea & vomiting? meds to help?
- indigestion or heartburn?

what are the important general exam techniques for GI assessment?
- good light
- empty bladder
- pt lying supine, arms at side, knees flexed slightly
- examine non painful side first
- visualize underlying anatomy
- watch pt face for signs of discomfort during the exam
what is expected findings for the abdomen?
- normal bowel sounds, soft and symmetric w/o distension
what is a scaphoid abdomen?
- malnourished, sees ribs
what is a protuberant abdomen?
- perturbating
what are signs of a non symmetrical abdomen?
- distension due to fluid or air
- budges
- hernia
- lumps or bumps
what are the for quadrants of the abdomen?
- Right upper quad
- Left upper quad
- Left lower quad
- Right lower quad
what are the two types of normal abdomen movements?
- surface motion of peristalsis or pulsations
when are hypoactive signs present?
- after surgery
what are the 4 types of abdomen sounds?
- normoactive
- hyperactive
- hypoactive

when palpating the abdomen what are we palpitating for?
- Palpate lightly for tenderness, muscle tone, and surface characteristics
- No tenderness should be present and the abdominal muscles should be relaxed
- also look for rebound tenderness and bladder distension
what is rebound tenderness?
-t o check for rebound tenderness, a nurse applies pressure to an area of your abdomen using their hands. They quickly remove their hands and ask if you feel any pain when the skin and tissue that was pushed down moves back into place.
