Centor criteria
A
One point each is assigned for
1. absence of cough
2. swollen, tender anterior cervical lymph nodes
3. T> 38C
4. tonsillar exudate and swelling
5. one point is added for patients ages 3-14 yo
one point is subtracted for patients over age 45
If the score is 0 or 1 the patient is at low risk for strep pharyngitis and does not need testing or antibiotic therapy
Patients with a score of 2-3 should be tested using rapid antigen test or throat culture
A positive test then warrants antibiotic therapy
Patients with a score of 4 or higher are at high risk and empiric therapy should be considered.
If the score is zero, neither testing nor treatment are needed
