Gastro part 2 Flashcards
How is 5-ASA (mesalamine) administered
PO, IM, IV and PR as suppository or enema. As a pro-drug that is converted to active form in the colon
What GI disease is 5-ASA (mesalamine) most useful for
UC, and mild to moderate crohn’s
What drug class is 5-ASA (mesalamine)
aminosalicylate
What is the MOA of 5-ASA (mesalamine)
Inhibition of leukotriene production, anti-prostaglandin and anti-oxidant effects
what bowel disease benefits more from antibiotics
Crohn’s disease benefits more from antibiotics than does UC
What antibiotics are commonly used to treat Crohn’s disease
metronidazole (flagyl) and ciprofloxin
What GI disease is rifaximin (ixifaxin) approved to treat
treatment of traveler’s diarrhea and hepatic encephalopathy
When is prednisone used in bowel disease
normally acutely in flares, but some patients are on constant low dose as maintenance therapy
What is the administration route of prednisone
PO, IM, IV, and PR (if bowel disease is limited to the anal and lower colon region)
What are side effects of prednisone
reduced resistance to infections, hypertension, hyperglycemia and possible diabetes mellitus, severe bone loss, avascular necrosis, cataracts, myopathy, thinning of skin, diminished wound healing, easy bruising, insomnia and mental status changes and possible adrenal suppression
what drug class is budenoside (entocort)
synthetic glucocorticoid
What drug class is azathiopine (immuran)
Immunomodulator
what is the MOA of azathiopine
Inhibits purine synthesis, resulting in an anti-proliferative effect and induction of apoptosis of T-cells
What drug class is infliximib (remicade)
immunomodulator - TNF inhibitor
What is the dosing schedule for infliximib (remicade)
IM of SQ every 2 weeks or every 4 weeks
