What do pretty much all anti-fungals target
ergosterol
What is the MOA of clotrimazole (lotrimin)
Binds to ergosterol molecules on the fungal cell membrane and causes membrane leakage during cell division
What are the indications for clotrimazole
Superficial mycoses (tinea, ring worm, yeast vaginitis)
What is the MOA of Nystatin (mycostatin)
Binds to ergosterol molecules on fungal cell membrane resulting in loss of membrane integrity death of the fungal cell
What are the indications for Nystatin (Mycostatin)
Superficial mycoses, oral candidiasis, esophageal and vaginal candida. Cryptococcus. Prophylaxis in at risk patients
How is nystatin administered
Either topically or oral suspension. Currently in trials for IV
What are the indications of Terbinafine (lamisil)
drug of choice for onychomycosis, superficial mycosis
What are possible side effects of terbinafine?
liver damage or elevation of liver enzymes
What is the administration route of fluconazole (diflucan)
topical, oral, vaginal, IV
What are the indications of fluconazole (diflucan)
superficial mycoses, systemic or meningeal mycotic infections by fungi such as Cryptococcus or Histoplasmosis
What is a cation with use of fluconazole
it can inhibit cytocrome systems used to process other drugs
What is the MOA of fluconazole
Inhibits fungal membrane synthesis (interrupts RNA)
What are the indications of amphotericin B
Systemic fungal infections such as systemic Candidadiasis, Histoplasmosis, Coccidiomycosis, Aspergillus and Blastomyces
What is the MOA of amphotericin B
Binds to ergosterol molecules on the fungal cell membrane and disrupts membrane integrity, resulting in the death of the fungal cell
What is the administration route of amphotericin B
PO, IV (May be used via intrathecal route for fungal meningitis or fungal brain abscess)
