Inflammation of gastric mucosa
Gastritis acute or chronic; diffuse or localized
Etiology of gastritis
Breakdown in gastric mucosal barrier allowing HCl acid and pepsin to diffuse back into mucosa results in tissue edema
Gastritis cause
Drug related (NSAIDs, corticosteroids) Diet: alcohol use, spicy foods Helicobactor pylori Environmental Diseases/disorders (autoimmune gastritis, crohn's, hiatal hernia)
Acute manifestations
i. Anorexia
ii. Nausea and vomiting
iii. Epigastric tenderness
iv. Hemorrhage
v. Self-limiting
Chronic manifestations
i. Similar to acute (lasts longer)
ii. Asymptomatic
iii. Pernicious anemia
1. Pt will need B12 injections
Gastritis: Diagnostic studies
a. Based on symptoms
b. History of drug or alcohol use
c. Endoscopy with biopsy
d. H. pylori testing
e. CBC (check for anemia)
f. Stool for occult blood
Treatment of acute gastritis:
identify cause supportive care (rest, NPO, IV fluids)
treatment of severe gastritis
NGT to monitor for bleeding
Lavage
Empty stomach
Risk for hemorrhage
Monitor VS, heme test vomitus; upper GI bleed strategies
Chronic gastritis
i. Evaluate and eliminate cause
ii. Antibiotics for H. pylori
iii. Cobalamin for pernicious anemia
iv. Lifestyle modifications
1. No smoking
2. 6 small meals/day; nonirritating food
