Gastric 4 Flashcards
What is intussusception?
Telescoping of bowl into itself.
Presents in children who are eating for the first time (increased peristalsis)
Sausaged shape mass in abdomen.
Current jelly stool.
What is difference between constipation and obstrication?
Obstrication, you can’t pass feces or gas.
Constipation you can pass gas.
Dermatitis herpetiformis associated with what malabsorption syndrome?
Celiac spru, due to forming of IgA
What is a pseudopolyp?
Found in Ulcerative Colitis.
A true polyp is growing above epithelial layer. A pseudopolyp is growing on top of the mucosa that is above the epithelium.
Extraintestinal assocation seen with Ulcerative Colitis
Ankylosing spondylitis (spinal joint disease) genetic assocation: HLA-B27***
Ischemic bowl disease (a thrombus causing hemorrhagic infarct of bowl) typically happens in what part of bowl?
Watershed areas: ie splenic flexure
Angiodysplasia
What is it?
Where?
Association?
Malformation of blood supply in intestines.
Most common in right colon (cecum) because it has the most surface area.
Assocation with CREST syndromes.
Diagnostic tool for pseudomembranous colitis
Clostridium difficile does not grow well in media (stool or blood culture).
Must detect toxin in the stool.
Meckel diverticulum rule of 2s
Diverticulum is a outpouching of structure.
2% feet from ileocecal valve
2cm in length
2yrs old or younger
2% of population
2% of carcinoid tumors occur in Meckel diverticulum
Tubular vs Villous adenomas
Which has higher likelihood of becoming cancer?
Villous
Because it has more dysplasia and larger
Peutz-Jeghers syndrome
Cancer assocation
It is autosomal dominant, multiple hamartomatous polyps in small intestines NOT ASSOCIATED with intestinal polyps, but it is associated with lung, pancreatic, breast, and uterus cancers.
Carcinoid syndrome most important indicator
Flushing of the face along with: diarrhea bronchospasm/wheezing fibrosis
Which has worse prognosis? Pancreatic carcinoma in head or body/tail of pancrease?
Body/Tail has worse prognosis because it is asymptomatic. Head will cause jaundice and increased conj. bilirubin and alk phos
Pancreatic Carcinoma is what type of cancer? Arises from what tissue?
Adenocarcinoma arising from duct epithelium
60% from head of pancreas
Islet cell tumors cause what symptoms?
Insulinoma (b-cell tumor) produces insulin causing hypoglycemia, sweating, hunger, confusion, insulin coma
