Pronator Teres
Origin: medial epicondyle of humerus and coronoid process of ulna
Insertion: lateral surface of radius
Action: pronate forearm, assist to flex elbow
Nerve: median C6, 7
Flexor Carpi Radialis
Origin: medial epicondyle of humerus
Insertion: bases of 2nd and 3rd metacarpals
Action: flex and abduct wrist, may assist to flex elbow
Nerve: median C6, 7, 8
Palmaris Longus
Origin: medial epicondyle of humerus
Insertion: flexor retinaculum and palmar aponeurosis
Action: tense palmar fascia, flex wrist, may assist to flex elbow
Nerve: median C6, 7, 8, T1
Flexor Carpi Ulnaris
Origin:
- humeral head: medial epicondyle of humerus
- ulnar head: posterior surface of proximal ulna (olecranon process)
Insertion: pisiform, hook of hamate and base of 5th metacarpal
Action: flex and adduct wrist, assist to flex elbow
Nerve: ulnar nerve C7, 8, T1
Flexor Digitorum Superficialis
Origin: medial epicondyle of humerus, ulnar collateral ligament, coronoid process of ulna, interosseous membrane and proximal shaft of radius
Insertion: sides of middle phalanges of fingers 2-5
Action: flex 2nd through 5th fingers, flex wrist
Nerve: median nerve C7, 8, T1
Flexor Digitorum Profundus
Origin: anterior and medial surfaces of of proximal ulna
Insertion: base of distal phalanges, palmar surface of fingers 2-5
Action: flex 2nd through 5th fingers, assist to flex wrist
Nerve:
- 1st and 2nd: median nerve C7, 8, T1
- 3rd and 4th: ulnar nerve C7, 8, T1
Flexor Pollicis Longus
Origin: anterior surfaces of radius and interosseous membrane
Insertion: base of distal phalanx of thumb
Action: flex thumb, assist to flex wrist
Nerve: median nerve C6, 7, 8, T1
Pronator Quadratus
Origin: medial, anterior surface of distal ulna
Insertion: lateral, anterior surface of distal radius
Action: pronate forearm
Nerve: median nerve C7, 8, T1
Brachioradialis
Origin: proximal lateral supracondylar ridge of humerus
Insertion: styloid process of radius
Action: flex elbow, assist to pronate and supinate forearm when movements are resisted
Nerve: radial nerve C5, 6
Extensor Carpi Radialis Longus
Origin: distal lateral supracondylar ridge of humerus
Insertion: base of 2nd metacarpal
Action: extend and abduct wrist, assist to flex elbow
Nerve: radial nerve C5, 6, 7, 8
Extensor Carpi Radialis Brevis
Origin: lateral epicondyle of humerus
Insertion: base of 3rd metacarpal
Action: extend and abduct wrist, assist to flex elbow
Nerve: radial nerve C6, 7, 8
Extensor Digitorum

Insertion: base of middle and distal phalanges of fingers 2-5
Action: extend 2nd through 5th fingers; assist to extend wrist

Insertion: base of 5th metacarpal
Action: extend and adduct wrist
Nerve: radial nerve C6, 7, 8
Supinator
Origin: lateral epicondyle of humerus, radial collateral ligament, annular ligament and supinator crest of ulna
Insertion: anterior, lateral surface of radial shaft
Action: supinate forearm
Nerve: radial nerve C5, 6, 7
Abductor Pollicus Longus
Origin: posterior surface of radius and ulna, and interosseous membrane
Insertion: base of 1st metacarpal
Action: abduct and extend thumb, abduct wrist
Nerve: radial nerve C6, 7, 8
