The four general areas
- level of consciousness
- Sensory and motor function
- Pupillary changes and extraocular movements
- Vital signs and pattern of respiration
Cardiac assessment
1 quality of rate and rhythm
2. Any abnormal sounds
3. Apical pulse
Respiratory : STEP 1
1- look for signs of injury and checks symmetry of chest expansion, note any difficulty breathing or use of axillary muscles
Respiratory step 2
Assess rate and quality of respiratory efforts
Look for cyanosis or pursed lip breathing
Respiratory step 3
Is the patient able to speak in complete sentences
Listen both anteriorly and posteriorly
Respiratory step 4
Palpate only if injury has occurred or any visible signs of irregularity in symmetry
6 Ps
Pain
Pallor
Paralysis
Parasthesia
Pulselessness
Poikilothermia
Pain
Compartment syndrome if unrelieved by pain meds
Pallor
Dusky, pale, compare unaffected extremity
Paralysis
Movement
Parasthesia
Tingling, numb
Pulselessness
Late sign in effective extremity
Poikilothermia
Compare temperature; cool May mean compartment syndrome
Abdomen assessment
- pain, nausea, vomiting, injury
- changes in appetite or bowel habits
- listen to bowel sounds
- listen for any bruits
Abdominal bruits are not
Normal
