what is a fracture
any break in the continuity of bone that occurs when more stress is placed on the bone than it is able to absorb
causes of fractures
- traumatic (fall)
- fatigue (prolonged, repeated stress)
- pathologic (weakened bone)
open fracture
- compound
- fractured bone penetrates skin
closed fracture
- simple
- does not break through the skin
clinical manifestations of fractures
- pain
- edema
- deformity
what are the three phases of bone healing
- inflammatory
- reparative
- remodeling
complications of fractures
- delayed healing
- bone growth impairment
- compartment syndrome
- fat embolism syndrome
delayed healing
- delayed union - bone pain and tenderness increase
- malunion - improper alignment
- nonunion - no healing 4-6 months post-fracture (poor blood supply, repetitive stress)
impaired bone growth
- pediatric consideration
- fracture through epiphyseal plate
- can delay future bone growth
compartment syndrome
- crush injuries and cast
- results from increased pressure within limited anatomic space
- tourniquet effect
- edema, loss/weakened pulses, PAIN
fat embolism syndrome
- fat molecules in the lung following long bone fracture or major trauma
what is osteomyelitis
an acute or chronic pyogenic infection of bone
what is the usual cause for osteomyelitis
bacteria
risk factors for osteomyelitis
- recent trauma
- diabetes
- hemodialysis
- IV drug use
- splenectomy
direct route of contamination for osteomyelitis
- open wound
- surgery/insertion of meta plates or screws
indirect route of contamination for osteomyelitis
- from bloodstream (most common)
- bacteremia
Osteomyelitis pathogenesis
- pressure increases within the bone
- causes local arteries to collapse
- leads to impaired healing and difficult to treat
local clinical manifestations of osteomyelitis
- tenderness, warmth, redness
- wound drainage
- restricted movement
- spontaneous fracture
systemic clinical manifestations of osteomyelitis
- fever
- positive blood culture
- leukocytosis
Osteomyelitis pharmacotherapy
- obtain culture
- empiric antibiotic therapy: nafcillin, cefazolin, vancomycin
- bacteria-specific therapy
