Exam 1 - Gastrointestinal Pharmacology Flashcards

1
Q

what is the ACVIM consensus statement on gastroprotectants?

A

consensus opinion on the rational administration of GI protectants to dogs and cats, with an emphasis on proton pump inhibitors (PPIs), histamine type-2 receptor antagonists (H2RAs), misoprostol, and sucralfate. These medications decrease gastric acidity or promote mucosal protective mechanisms, transforming the management of dyspepsia, peptic ulceration, and gastroesophageal reflux disease

judicious use of acid suppressants is warranted considering recent studies that have documented adverse effects of long-term supplementation of PPIs in people and animals

basically we over use PPI & should do better when considering what drugs are indicated & why

Q

what is the mechanism of action of famotidine?

A

H2 receptor antagonist - works at the H2 receptors in the parietal cells competitively inhibiting histamine to reduce gastric acid production

Q

what is the mechanism of action of omeprazole?

A

proton pump inhibitor - inhibits the H+K+-ATPase pump of the parietal cells to decrease gastric acid production

Q

what is the mechanism of action of misoprostol?

A

PGE1 analogue - prostaglandin analogue

provides mucosal defense through decreasing gastric acid production & increasing bicarbonate production

Q

what is the mechanism of sulcralfate?

A

gi mucosal protectant - reacts with HCl acid & forms a paste on damaged lesions of gi mucosa

Q

when is sucralfate indicated?

A

there has to be damaged lesions in the gi tract for it to bind & actually work - if there is no damaged lesions, no point in giving it
