Amoxicillin 1st if not resolved = Amox + Clav. Recurrent = Tubes!
Pen Allergy? Cefdinir, Axith
SX: unilateral ear pain w/relief by pullon on ear + red mem + loss of light reflex
Unilateral ear pain w/pain on palpation
Otitis Externa! prob see angry canal due to staph aureus or pseudomonas.
Tx for Otitis Externa?
sx?
do nothing! will resolve spontaneously. If not resolveing can give Cipro ear drops
sx: unilateral ear pain w/pain on palpation
Purulent bilateral nasal discharge w/sinus tenderness & pain on facial tap. Dx?
Sinusitis
What would radiographs show for sinusitis?
CT: opacification
XR: air-fluid levels
^dont need to get any of these! dx is clinical!
Treatment of Sinusitis if:
- > 7d +cough
- <7d but no cough
- amoxicillin = bacterial!
2. wait it out! = viral
Person w/acute otitis media + mastoid swelling in a person with ear tubes. Dx? Tx?
Mastoiditis, Surgery
Sore throat? DDx?
GBS infection or Mono!
Tx for sore throat
Amoxicillin-clavulante
Sore Throat work up?
Strep Test if + treat if - do culture! if both negative wait! its viral. if one positive = amox-clav
Foreign Body sx? Tx?
unilateral pain,discharge!
Kid: remove object
Adult: Lidocaine + remove bug!!
Infant who turns blue with feeding & Pink with Crying. Baby snores. Dx? Tx?
try to pass catheter or do fiber-optic scope = Choanal atresia!
Tx w/ surgery to remove tonsils or open passage
Recurrent vertigo lasting min to hrs. +/- hearing loss, tinnitus. Dx? Tx? Cause?
MENIERES DZ = increase volume and pressure of endolymph
Tx w/ diuretics and carbonic anhydride inhibitors
Vertigo triggered by changes in head position lasting min. Dx? Tx?
Benign positional vertigo = no hearing loss. Tx w/ meclizine
Dx= dix-hallpike causes nystagmus
