How is status epilepticus defined?
A
A continuous seizure lasting more than 5 minutes
OR having repeated seizures without full recovery in between

How should it be managed?
A
Call for help
Secure airway
Give oxygen
Check blood glucose and establish IV access
If vascular access: lorazepam IV (0.1mg/kg)
If no vascular access: buccal midazolam or rectal diazepam
At 15 minutes repeat lorazepam, call for senior help, prepare phenytoin
At 20 mins start phenytoin IVI 18mg/kg over 20 mins, monitor ECG or if already on phenytoin given phenobarbital over 5mins, call anaesthetist
At 40mins rapid sequence induction of anaesthesia with thiopental

Glucose U&E FBC Calcium, magnesium ABG ECG
