Bulimia Nervosa
Engage in repeated episodes of binge eating followed by compensatory behaviors (i.e. self induced vomiting, misuse of laxatives/diuretics/other meds, fasting, or excessive exercise
Anorexia Nervosa
Refuse to maintain minimally normal weight for height and express intense fear of gaining weight
Binge Eating
Repeated episodes of binge eating, after which they experience significant distress
When are antidepressants effective for anorexic patients?
Mood changes begin after a patient reaches at least 90% of optimal weight
Anorexia: Sx
Low weight, amenorrhea, yellow skin, lanugo, cold extremities, peripheral edema, muscle weakening, constipation, abnormal lab values, cardiovascular abnormalities (hypotension, bradycardia, heart failure), impaired renal function, hypokalemia, anemic pancytopenia, decreased bone density
Common comorbidities
OCD, anxiety, social phobia, substance abuse
Refeeding Syndrome
Treatment complication involving metabolic alteration in serum electrolytes, vitamin deficiencies, and sodium retention
Overgeneralization
A single event affects unrelated situations (i.e. “he didn’t ask me out. It must be because I’m fat.”)
All-or-nothing thinking
Reasoning is absolute and extreme, in mutually exclusive terms of black or white, good or bad (i.e. “If I have one popsicle, I must eat five”)
Catastrophizing
The consequences of an event are magnified (i.e. “If I gain weight, my weekend will be ruined”)
Personalization
Events are overinterpreted as having personal significance (i.e. “I know everyone is watching me eat”)
Emotional Reasoning
Subjective emotions determine reality (i.e. “I know I’m fat because I feel fat”)
Bulimia Nervosa: Sx
Normal to slightly low weight, dental caries, tooth erosion, parotid swelling, gastric dilation/rupture, calluses/scars on hand, peripheral edema, muscle weakening, abnormal lab values (electrolyte imbalance, hypokalemia, hyponatremia), cardiovascular abnormalities (cardiomyopathy, electrocardiographic changes), cardiac failure
Which med is contraindicated for pts who purge?
Bupropion (Wellbutrin) - increased risk for seizures
