Bipolar II
Hypomanic episodes alternate with major depression; psychosis not present
Bipolar I
At least 1 episode of mania alternates with major depression; psychosis may accompany manic episode
Cyclothymia
Hypomanic episodes alternate with minor depressive episodes (for at least 2 years); individuals with this tend to have irritable hypomanic episodes
Rapid Cycling
4 or more mood episodes in a 12 month period
First-Line Tx for Bipolar Disorder
Lithium
Flight of Ideas
Nearly continuous flow of accelerated speech with abrupt changes from topic to topic that are usually based on understandable associations or a play on words
Grandiosity
Inflated self regard
Acute Phase
Injury prevention
Continuation Phase
4 to 9 mo; relapse prevention
Maintenance Phase
Relapse prevention with limitation of severity/duration of future episodes
Lithium: Maintenance Blood Levels
0.4-1.3 mEq/L
Lithium: Therapeutic Blood Levels
0.8-1.4 mEq/L
Lithium: Sx of Toxicity
Nausea, diarrhea, polyuria, tremors, lethargy, slurred speech, ataxia, confusion, coma
Lithium: Long-Term Risks
Hypothyroidism, kidney’s impairment to concentrate urine
Seclusion Protocol
Order must be reviewed & rewritten every 24 hours; observation/documentation every 15 minutes; offer food/fluids every 30-60 minutes; toileted every 1-2 hours; vital signs every 1-2 hours
