Hypervigilance
Overly alert
Delirium

Dementia
Progressive deterioration of cognitive functioning and global impairment of intellect with no change in consciousness
Most common cause of dementia in older adults
Alzheimer’s Disease

Irreversible, progressive, and not secondary to any other disorder (i.e. AD, vascular dementia)
Secondary Dementia
Occurs as a result of some other pathological process (i.e. AIDS dementia complex, Korsakoff’s syndrome)
Confabulation

Loss of sensory ability to recognize objects
Disturbance in Executive Functioning
Loss of planning, organizing, abstract thinking skills
Alzheimer’s Disease: Stage 1(mild)
Loss of energy, drive, initiative; difficulty learning new things
Alzheimer’s Disease: Stage 2 (moderate)

Alzheimer’s Disease: Stage 3 (moderate to severe)
Severe agnosia, advanced apraxia, wandering; client often in need of total care; agitation, violence, paranoia, delusions are common
Alzheimer’s Disease: Stage 4 (late)
Agraphia, hyperorality, hypermetamorphosis, blunting of emotions, visual agnosia

Inability to read or write
Hyperorality
The need to taste, chew, put everything in one’s mouth
Hypermetamorphosis
