Dopamine - Action (5)
A
- Dose dependent sympathomimemtic- 1-2 mcg/kg/mins stimulate dopaminergic receptors > cerebral/renal/mesenteric vasodilation- 2-10 mcg/kg/mins B1 effects (mainly inotropic) w/ alpha beginning (vasoconstriction)- 10-20 mcg/kg/mins alpha effects ^renal/mesenteric/peripheral arterial vasoconstriction (increased SVR)- 20+ mcg/kg/mins - alpha effects similar to norepi w/ peripheral vasoconstriction/tachycardia

Dopamine - Indications (3)
A
- Cardiogenic shock- Other Shock after adequate fluid resuscitation- symptomatic bradycardia unresponsive to atropine or external PM

Dopamine - Side Effects (3)
A
- Tachyarrhythmias- HTN > MI- Tissue necrosis w/ infiltration

Dopamine - Contraindications (4)HUMP
A
- Hypovolemia prior to fluid resuscitation- Uncorrected tachyarrhythmias- Monamine oxidase Inhibitors (MOI)- Pheochromocytoma (precipate HTN crisis)

Dopamine - Precautions (4)
A
- Monitor VS/EKG- Stop infusion if BP >90 mmHg- Doesn’t work w/ Bicarb due to Alkaline deactivating dopamine- avoid extravasation => tissue necrosis, inform RN
