Herpes Simplex Virus
-Enveloped, linear, double stranded
-Cowdry bodies
Trans: Sex, saliva, vertical transmission (torch)
HSV1: upper half
-Gingiviostomatitis to cold sores
-Keratoconjuctivitis (snake like on lamp exam)
-Temporal lobe encephalitis
-Latent in trigeminal gang
-Dew drop rose petal vesicular RASH (on fingers, dentists = whitlows)
HSV2: lower half
-Painful, genital lymphadenopathy
-Dormant in sacral gang
-Meningitis
PMS
TEST: PCR (Tzank smear, but no longer used)
TX: Acyclovir or Valacyclovir (for breakouts, not curable)
Varicella Zoster Virus
-Enveloped, Chicken Pox
-Fever, Headache
-Vesicular rash “dew drops on rose”
-Adult comp: pneumonia, encephalitis
Trans: respiratory drops, torches infection
Tzank smear
LIVE attenuated vaccine
TX: Acyclovir
Shingles! Very painful (possible vision loss, vesicles)
-Zoster vaccine (live)
-Can give to HIV Pts if CD4 count >200
TX: Acyclovir, Famciclovir, Valacyclovir (FAV)
Cytomegalovirus
-Enveloped, remain latent
-Torches infection
-Blueberry muffin rash
-Jaundice, hepatosplenomegaly
-Hearing loss (deafness)
-Seizures
-Babies: 80-90 asymptomatic
-#1 cause of retardation and hearing loss
-2nd trimester, highest risk
-Pneumonia in transplant pts
-CD4 <50, AIDS pts: pizza pie retinitis (+ linear ulcers in throat, colitis)
TX:
-Ganciclovir
-Foscarnet (UL97 mutation resistant)
Negative monospot test (not mono)
Epstein Barr Virus
Causes MONO
-Fever, lymphadenopathy
-Splenomegaly
-Pharyngitis, tonsille exudates
Trans: Saliva
Latent in B cells
If given penicillin class = causes RASH
Cancer risk
-B cell lymphoma (RS cells, owl eyes)
-Burkitt lymphoma (jaw lesion swelling)
-Naso carcinoma
HIV pts: hairy leukoplakia
Mono spot test
TX:
-Avoid contact sports (spleen rupture)
-Supportive therapy
Human Herpesvirus 8
Kaposi’s Sarcoma
-Violacious (Violet) lesions on nose, extremities, mucous membranes
-Can be in GI tract or hard palate in mouth
Dysregulation of VEG-F
B cell lymphoma risk
Trans: sexual contact, kissing, men sex w. men
-Russian, African (SKAMR)
TX: anti retro viral therapy if HIV+
Hepatitis B Virus
Circular genome, partially double stranded
-Reverse transcriptase
Trans: Sex, blood products (needles)
-TORCHES infection
Can be passed onto baby if mixing of blood
-Polyarteritis nodosa
-RASH
-Arthralgia
-Reduced GFR, HTN (kidney damage)
HIGH ALT
-Liver cancer
-Hep D needs Hep B to become infectious
TX: Lamivudine (NRTIs)
-Baby: Immunoglobulins and Hep B vaccine
Adenovirus
-Tonsilitis, infection of adenoids
-Hem cystitis
-Conjuctivitis (pink eye)
Trans: respiratory drops, fecal oral route
-Babies, military, public pools
Military gets live vaccine
