Diseases Of The Peripheral NS Neuro/Patho Lecture 9 Flashcards
What are neuropathies
Disease or dysfunction of one or more peripheral nerves, typically causing numbnes or poor weakness
What is Charcot-Marie-tooth disease
Hereditary cause
Neuropathy/progressive peroneal muscular atrophy
Involves peroneal nerve and affects muscles in the foot and lower leg
What are the pathologies for a CMT
Hypertrophic ONION bulb formation
Onion bulb formation creates palpable, enlarged peripheral nerves
What are the clinical features of CMT
Pes cavus
Hammer toes
Inverted champagne bottle
(T/F) there is no cure for CMT
True
What is diabetic neuropathy
METABOLIC CAUSE
associated w/ diabetes mellitus
Risk factor: hyperglycemia
What are the clinical feature of diabetic neuropathy
Insidious onset
Sensory loss
Burning pain
Parathesias that are more profound at night in feet
Walking on cotton
What is myasthenia gravis
Autoimmune disorder
Most common neuromuscular disorder
Fluctuating motor weakness involving:
- Ocular
- bulbar
-Limb
- and/or respiratory muscles
What are the physical presentation of someone w/ myasthenia gravis
Eyelids muscles and those moving the eyes are usually the first affected
Facial expression- appears to snarls- lips wont close
- could have difficulty swallowing
What is Guillian barre syndrome
Autoimmune disease
Most common inflammatory demyelinating polyradiculoneuropathies
Starting distally, progressing SYMMETRICALLY PROXIMAL
What are the symptoms required for diagnosis for GBS
Progressive weakness in more than 1 extremity
Loss of DTR
What are the CSF feature of GBS
CSF protein levels increased after 1 week
What are the electrodiagnostic features of GBS
Slow nerve conduction velocity
How fast is remyelination/healing after nerve injury ?
Rate of peripheral nerve recovery: SLOW
What is the rate of axonal regeneration
1-2mm/day
What is the rate of long term recovery for remyelination/healing after nerve injury (how many inches per month)
1-2 inches per month
What are the PT implications for GBS
Most patients return to previous level of functioning
Facilitate muscle contractions whole MONITORING OVERUSE AND FATIGUE
Time is important factor of healing
(T/F) Myasthenia gravis is common in both men and women
True
Women < 40
Men > 60
What diseases affect the peripheral nervous system
Charcot Marie tooth disease
Diabetic neuropathy
Myasthenia gravis
Guillian barre syndrome
Which peripheral nerve maintains blood nerve barrier
Perineurium
The peripheral nervous system is made up of how many pairs of spinal nerves and pairs of cranial nerves
31 pairs of spinal nerves
10 pairs of cranial nerves
What are hallmarks signs of PNS lesions
Sensory loss
Weakness
What kind of neuropathy has multiple nerves involved and has glove and stocking patterns
Polyneuropathy
What kind of neuropathy is diabetic neuropathy
Polyneuropathy
