Diltazem Flashcards
What is another name for Diltazem?
Cardizem
What is the class of Diltazem?
Calcium channel blocker (Class IV antiarrhythmic)
What is the action of Diltazem?
Vascular dilation slows rapid ventricular rate associated with A-fib/A-flutter. Has a negative inotropic effect. Can be used for angina
What are the indications for Diltazem? (2)
- Stable narrow complex tach if uncontrolled or unconverted by adenosine or vagal maneuvers or in PSVT
- To control ventricular rates in patients w/ A-fib/A-flutter
What are the contraindications for Diltazem? (6)
- Hypotension
- CHF
- Cardiogenic shock
- V-tach
- Wide complex tach
- A-fib/A-flutter with WPW
What is the onset for Diltazem?
3 min
What is the duration of Diltazem?
1-3 hours
What is the peak of Diltazem?
7 minutes
What is the 1/2 life of Diltazem?
2 hours
What is the dose of Diltazem?
Narrow-complex tach 15-20 mg IV bolus (.25mg/kg) over 2 minutes. An additional bolus dose of 20-25mg (.35mg/kg) over 15 minutes
PRN maintenance drip of 5-15 mg/kg can be administered, titrate to HR
What are the side effects of Diltazem? (6)
- N/V
- Dizziness
- headache
- bradycardia
- heart blocks
- asystole
What is the description of Diltazem?
Calcium-ion antagonist causes a relaxation of vascular smooth muscle and slows electrical conduction through the AV node
What is a precaution with Diltazem?
Can cause systemic hypotension. Monitor BP. can use calcium chloride to prevent hypotension effects of calcium channel blockers or calcium channel blockers OD
