disruption of normal skin pattern due to skins contact with moisture, friction, urine, feces
what are the 3 type
allergic

signs and symptoms of irritant
shiny red patches in diaper region, NOT on the folds of the skin
signs and symptom of cutaneous fungal infection
beefy red rash with papules and pustules - satellite lesions
red plaques
in folds of the skin
risk factors
irritation and friction - bowel frequency
type of diaper used? - cloth and disposable both cause
comorbid condition - atopic dermatitis
chemicals - wipes
occlusion and humidty
red flags
no improvement over a week or worsens pus or signs of infection acute onset systemic symptoms N/V, fever, diarrhea
goals of therapy
relieve symptoms
resolve dermatitis
prevent complications and recurrence
non pharms
dry area by patting gently baby wipes that are alcohol and fragrance free diaper changes barrier product bath daily in luke warm water
what are 3 water impermeable barriers
petrolatum
anhydrous lanolin
dimethicone
what does zinc oxide do
mild antiseptic and astringent (drys) low concentrations (15%) for prevention high concentrations (25%) for treatment
how cna you use a barrier product for prevention
with each diaper change
petrolatum or zinc oxide 15%
how can you treat diaper derm
zinc oxide 25%
how can you treat a candidal infection
apply antifungal twice daily for one week
apply before the barrier product
treatment if all others ineffective
topical corticosteroid 0.5-1% three times daily for one week
