- can be used as evidence in a legal settlement or lawsuit

- patient should sign informed consent form
where should the mdeical alert be placed within the patient record
if it is paper, it should be placed on the inside of folder to maintain confidentiality
should contain:
- premedication
- drug allergies
- latex allergy
- pregnancy
what is proper recording of dental procedures
treatment plan
- plan of care should be recorded on treatment plan
- will be sequenced to address problems identified during examination
progress note
- will have details of what was accomplished
- date, tooth number, treatment and communication with patient
