Diabetes management and therapeutics Flashcards
What is endogenous insulin?
Produced in islets of Langerhans
structure of insulin
Polypeptide chains A and B connected by 2x disulphide bridges
How is insulin formed
Preproinsulin formed in RER
Cleaved into proinsulin
Transported to Golgi and packaged into granules
Equal amounts of proinsulin are cleaved into C peptide and insulin
How do you measure insulin production
C-peptide
Characteristics of rapid insulin
Takes 15 mins to onset, lasts 2-4h
Examples of rapid insulin
Aspart and lispro
Characteristic of short insulin
30-60min onset, lasts 6-8h
Examples of short insulin
Soluble, regular insulin
Characteristics of intermediate insulin
1-2h onset, lasts 11-18h
Examples of intermediate insulin
NPH (isophane insulin)
Characteristics of long acting insulin
1-2h onset, lasts 20-24 h
Examples of long insulin
Glargine, deter
Why does intermediate insulin take so long to act?
Administered with protamine
The crystals dissolve slowly = delayed absorption
Biphasic regimen
2x intermediate insulin at morning and evening meal
short acting insulin when needed
Why does insulin demand increase after surgery?
Cortisol increases
