diabetes insipidus Flashcards
diabetes insipidus definition
the inability to produce dilute urine
central pathophysis
The hypothalamus doesn’t produced ADH
central causes
acquired, tumors, trauma, infection
central causes
acquired, tumors, trauma, infection
nephrogenic pataphysics
kidney doesn’t respond to ADh
nephrogenic causes
lithium, congenital nephrogenic diabetes insipidus, PKD, renal ischaemia, hypercalcaemia, hypokalemia, renal tubular acidosis, CKD
gestational diabetes insipidus pathophysis
Vasopressinase breaks down ADH
disogenic diabetes
chronic meningitis, MS
diabetes insipidus symptoms
dehydration (fatigue, dizziness), weakness, space-occupying lesions (headaches, visual changes, seizures), polyuria, polydipsia
investigations of diabetes insipidus
U+E (hypernatremia), low urine osmolality (U:P <2:1), BM normal, ca = normal, high ser osmolality, 24 hour urine diary 3+L dehydration test
dehydration test results
normal = high -> high
central = low -> high
nephrogenic = low -> low
diabetes insipidus DD
diabetes, psychogenic polydipsia, diuretic overuse, UTI, hypercalcaemia, Cushing syndrome
diabetes insipidus treatment
desmopressin (central/gestational), nephrogenic (thiazide diuretics) + indomethacin
