environment (cold air, hot)

teething
stress
allergic eczema
suspect food allergy
immediate reaction (type I): blood test for specific IgE antibodies (RAST), skin prick test
late reaction (type IV): dietary elimination for 6 - 8 weeks

topical steroids (hydrocortisone)
