DSM Criteria
Must have 5 or more of the following symptoms ( SIGECAPS) during the same 2-week period, representing a change from previous functioning. At least one must be either:
Depressed mood
Loss of interest or pleasure
What does the “S” in SIGECAPS stand for (first S)?
Sleep disturbance (too much or too little)
What is required for a diagnosis of MDD in terms of core symptoms?
At least 5 symptoms, including either depressed mood or loss of interest, present for at least 2 weeks.
What does the S in SIGECAPS (first S) stand for?
Sleep disturbance — either insomnia or hypersomnia
What does the I in SIGECAPS stand for?
Interest loss — loss of interest or pleasure in most activities (anhedonia)
What does the G in SIGECAPS stand for?
Guilt or feelings of worthlessness — excessive or inappropriate
What does the E in SIGECAPS stand for?
Energy loss — fatigue or loss of energy nearly every day
What does the C in SIGECAPS stand for?
Concentration problems — diminished ability to think or concentrate
What does the A in SIGECAPS stand for?
Appetite changes — significant weight loss or gain, or change in appetite
What does the P in SIGECAPS stand for?
Psychomotor changes — either agitation or retardation, observable by others
What does the S in SIGECAPS (last S) stand for?
Suicidal thoughts — recurrent thoughts of death, suicidal ideation, or attempt
How many SIGECAPS symptoms are required for an MDD diagnosis?
At least 5 symptoms, including either depressed mood or loss of interest, lasting for 2+ weeks
Excessive guilt
Feeling too guilty for something minor or neutral
Inappropriate guilt
Feeling guilty about things not your fault at all
Depressed mood
Feeling sad, empty, hopeless most of the day, nearly every day
Criteria
Must cause clinically significant distress or impairment in social, occupational, or other areas of functioning.
Differential
Cannot be better explained by substances, medical conditions, or other psychiatric disorders (e.g., bipolar).
