- CVS prophylaxis in DM / Atherosclerotic pt

What is the GENERAL MOA of Ramipril?
A
Antagonises RAAS system

What is the IN-DEPTH MOA of Ramipril? (3 steps)
A
- Binds to + Inhibits ACE
- Prevents Angiotensin I –> Angiotensin II conversion
- No more Angiotensin II, so instead:
- Vasodilation
- Reduced inflammation

A
Oral

What are the Min / Max Doses for Ramipril?
A
- Min: 1.25mg OD
- Max: 10mg OD

What are the Monitoring Requirements for Ramipril?
A
Check Renal function + Electrolytes b4 starting / increasing dose / during tx

What are the Side fx of Ramipril? (6 things)
A
- Dry cough
- Dizzy (esp when you get up)
- Headaches
- Vomiting / Diarrhoea
- Rash
- Blurred vision

What advice should you tell a patient taking Ramipril?
A
First dose to be taken at bed time

What are the CI for taking Ramipril? (2 things)
A
- Angioedema
- Hx of angioedema w previous ACEi Tx

When should Ramipril be used with Caution? (7 things)
A
- Diuretics
- DM (risk of hypoglycaemia + hyperkalaemia)
- Niggas
- PVD
- Primary aldosteronism
- Aortic / Mitral stenosis (risk of hypotension)
- Hypertrophic cardiomyopathy
