What are the common indications for Aldosterone agonists?
A
- Ascites and oedema due to liver cirrhosis: spironolactone used first line
- Primary hypoaldosteronism

What is the mode of action?
A
- Aldosterone is a mineralocortcoid produced in adrenal cortex
- Acts on the distal tubules of kidney.
- Increase Na uptake and hence water
- Elevates BP
- Retains K
- Spironolactone inhibits effects of Aldosterone
- Hence increases water and Na excretion

What are some important adverse effects?
A
- Hyperkalaemia
- Gynecomastia
- Liver impairment
- Stevens Johnson syndrome
- T cell mediated hypersensitivity reaction

Special warnings?
A
Contraindicated in patients with:
- Severe renal disease
- Hyperkalaemia
- Addison’s
- Pregnant/lactating women

What are the important interactions to consider?
A
- Combination with other potassium elevating drugs: ACE, ARB
- Potassium supplements
