The behaviour can range from causing slight embarrassment to criminal acts.

  • Reporting culture: Culture/climate where people are prepared to report their errors and near-misses
  • Just culture: People encouraged to provide essential safety related information. It must be clear what is acceptable and unacceptable behaviour
  • Flexible culture: Organisation is able to reconfigure itself to adapt to changes.
  • Learning culture: Organisation has the willingness and competence to draw the right conclusions from its safety information system will ti implement major reforms.
