Principles of a PTW

A

Relevant to the job at hand, people working

Specify the period of validity (24 hours max)

Only work specified to be carried out

Authorising Officer must ensure measures taken before signing

Responsible (competent) person must sign as well

Cannot be the same as the authorising officer

Officer must remain present until a new officer takes over, who has been briefed and signed as well

Closing of the work, responsible person must notify the officer

  2. Confirm location, number of people and nature of incident
  3. Rescue team and medical team mustered
  4. Area restricted
  5. PPE checked and used

  8. Situation prior to entry evaluated to ensure safety of those performing rescue
  9. Rescue performed in line with ship contingency plan
