A
  • A doctor, dentist, independent prescriber or independent nurse prescriber
  • A supplementary prescriber acting in accordance with clinical management plan
  • A person acting in accordance with the direction of a prescriber entitled to prescribe CDs (e.g. pharmacists)

  • Diamorphine
  • Morphine
  • Pethidine

A

How should solid dosage forms be denatured?

A
  • Grind or crush before adding to denaturing kit, using small amount of water to avoid release of particles into air
  • Where a denature kit is not available, crushed drug is added to warm soapy water and mixed to dissolve. Mixture is poured into a suitable product and placed in pharma waste container and sent for incineration

A

1) Yes, IF Sch 2. For Sch 3, it would be GOOD PRACTICE to have another member of staff witness denaturing
2) Entry should be made in CD register for Sch 2 CDs

How should liquid dosage forms be denatured?

A
  • Pour into an appropriately sized denaturing kit
  • Where one is not available, pour liquid into a suitable product (diluent) and pour into an appropriate waste disposal bin
  • Bottles can be rinsed using denaturing kit liquid and then recycled appropriately

How should ampoules or vials be denatured?

A
  • For liquid containing ampoules- open ampoule and pour into denaturing kit and then same as a liquid formulation
  • For powder containing ampoules- open ampoule and add water to dissolve powder inside. Resulting mixture can be poured into denaturing kit

How should patches be denatured?

A
  • Remove backing and fold the patch over itself

- Place into waste disposal bin or denaturing kit

A

1) Patient details - name and address
2) CD drug details - name, form strength, dose, quantity (in WORDS AND FIGURES)
3) Date (Valid for 28 days)
4) Prescriber signature and details - address
