What are the four main ways to structure a consultation?

- Introduction
- Information gathering
- Analysis

What does the introduction entail?
A
- Introduce myself (#hellomynameis)
- Confirm the patients identity
- Purpose of the consultation – WHY are you having it
- Structure of the consultation – HOW its going to run
- Invite patient to highlight any points this would like to be discussed
- Negotiate a shared agenda

What does the information gathering entail?
A
- WWHAM (open questions)
- W - Who
- W – What are the symptoms
- Anything make them better or work
- Had them before? What helped?
- H – How long
- A – Action taken
- M – Medications
- Prescribed
- OTC
- Herbal / vitamins / alternative
- Allergies or intolerances – gluten, lactose
- Lifestyle – smoker, alcohol, occupation,
pregnancy, vegan or vegetarian, social support - Medication support: NOMAD, Carers, help with
their medication (e.g. relatives) - Health conditions
- Pregnancy, breastfeeding

What does the analysis entail?
A
- Closed questions
- Causes of those symptoms?
- Differential diagnosis
- Any reg flag symptoms that require the patient
to be referred to another HCP - Has any of the medication the patient is taking
contributed to their symptoms? - Can I sell the OTC product?
- Drug-drug interactions
- Patient-drug interaction – any reason the
patient cant have the medication - Red flags
- What product is best for the patient
- What matters to the patient

What does the close entail?
A
- Re-cap important points
- Safety netting – what to do if things go wrong /
unable to follow plan / things don’t improve - Opportunity for patient to ask questions
- Provide point of contact if any further questions
- Follow up plan
