Congenital heart Flashcards
Cardiac development fairly complete by
8 weeks
Ductus Venous
Carries oxygenated blood from the placenta though the liver into the inferior vena cava–becomes ligament in 3-7 days
Formane ovale
Flapped opening in atria that allows blood to shunt right to left—closes immediately
Ductus arterious
connection between aorta and pulm artery that allows blood to shunt from plum artery to the aorta–functionally closes over 10-15 hrs and structuraly over 72hrs
What med closes a PDA
idomethican
1st degree heart block
prolonged PR–not on time but always present
2nd degree heart block–type 1
PR gets progressively longer until drops
2nd degree heart block type 2
P wave on time but randomly drops
3rd degree heart block
p waves not in conjuct with QRS
Four defects of TOF are
- VSD
- Overriding aorta
- Thickened Right Ventricle wall
- Pulmonary stenosis
TX of TOF Spell
- Knees bent to chest
- O2
- sedation
- volume for expansion
- phelenephrin for prolonged spell-(alpha agonist)
- Propanolol for prevntion of spells
goal of TOF spell tx
decrease tone of indafibular muscle or increase SVR to stop right to left shunt
Blue spells of TOF related to what defect?
Pulmonary stenosis–blood flow decreased to lungs–blood cant get oxygenated
best indicator of chronic right to left shunt?
hematocrit–venous blood going into systemic circulation—body will great more red blood cells to try to have more places for oxygen and nourish body
S/S of severe pulmonary stenosis
Cyanosis with activity and intolerance with activity, no symptoms at rest, increase in right ventricular pressure
