Symptoms of meningitis
Fever, stiff neck, photophobia, some brain function disturbance
Causative organisms of acute meningitis
Bacterial:Neisseriameningitidis(Gram-ve),Streptococcuspneumoniae (Gram +ve)
§ Neonates: Group B Strep, Listeria monocytogenes, E. coli
§ Elderly: Group B Strep, Listeria monocytogenes
Viral:Enterovirus(coxsackie,echovirus),mumps,HSV2
§ Viral presents the same as bacterial (but often less severe)
Cause and presentation of chronic meningitis
Presentswithheadachesformonths,
caused by TB or cryptococcus
How does meningococcal meningitis present ?
Non blanching rash
Can present with septicaemia as well- which presents with hypovolaemia, coagulopathy, acidosis and multi organ failure
What is aseptic meningitis and what is the main causative organism
Meningitis with negative CSF
Enteroviruses (e.g. Coxsackievirus group B and echoviruses) are responsible for 80-90% cases
Symptoms of Encephalitis
Confusion, fluctuating consciousness
Main causative organisms of encephalitis
HSV1 but West Nile virus is increasing
Toxoplasmosis, immunocomprimsied
Symptoms of brain abcess
Headache, vomiting, seizure, swinging fever
How brain abscesses occur
Through local extension- otitis media
Haematogenous spread- endocarditis
Who are at risk of getting bacterial meningitis
o Overcrowding,youngadults/veryyoungorold
o N.meningitidis:Complementdeficiency,hyposplenism(susceptibleto
encapsulated organisms), hypogammaglobulinaemia
o S.pneumoniae:Complementdeficiency,hyposplenism,immunosuppressed
(alcoholic), infection (pneumonia), entry #, previous head trauma w/ CSF leak
CSF of bacterial infection
Turbid, low glucose, high WBC, polymorphs (neut, bass, eso)
CSF of viral infection
Clear, normal glucose, high WBC, mononuclear (lymph and mono)
CSF of TB infection
Clear/turbid, low glucose, high WBC, mononuclear, high protein
Treatment of Bac Meningitis
Resuscitate! IV ceftriaxone and corticosteroids (cover Listeria with ampicillin)
