what is CKD?
- Abnormal kidney structure or function
- present for >3 months with implications for health
define CKD with numbers
egFR < 60 ml/min/ 1.73m2 for 3+ months
CKD classified- what is stage 1?
egFR - 90 or higher
pretty much normal
CKD classified- what is stage 2?
egFR- 60-89
kidneys work less than normal
CKD classified- what is stage 3a ?
egFR- 45-59
kidneys work even more less than normal
CKD classified- what is stage 3b ?
egFR- 30-44
kidneys work even more less than normal
CKD classified- what is stage 4?
egFR- 15-29
kidneys close to not working
CKD classified- what is stage 5?
egFR- less than 15
most severe, kidneys close to not working or stopped
what are the 4 parameters of CKD?
- Creatinine
- Age
- Gender
- Ethnicity
what are the risk factors for CKD?
- DM
- HTN
- Glomerulonephritis
- nephrotoxic drugs
- PKD
- male
- smoking
describe the pathology behind CKD
- ~ 7 mil nephrons but in CKD many damaged so descreased GFR
- therefore increased burden on remaining nephrons
- compensatory RAAS to increase GFR but raised transglomerular pressure= shearing and loss of Bm selective permeability.
- leads to preoteinuria
- angiotensin 2 up regulates TGF- B and plasminogen activator- inactivator 1
- causes mesangial ( supportive tissue scarring)
what are the symptoms for CKD?
- early on asymptomatic- due to loss of nephrons- reserved supply
what are the symptoms of CKD due to?
- substance accumulation and renal damage ( diabetic nephropathy)
what are4 complication of CKD?
-anaemia
-neuropathy
-cvd
-osteodystrophy
how is CKD diagnosed?
- U+E
- FBC
- Urinalysis
- renal USS
- GFR function staging
- albumin: creatinine ratio
