- increase in size of prostate gland with no malignancy
- normal with ageing
what are the risk factors for benign prostate hyperplasia?
- increasing age
- ethnicity - afro Caribbean- increased testosterone
-family history - diabetes
- heart disease
- obesity
what is protective to benign prostate hyperplasia?
castration
what is the pathology behind benign prostate hyperplasia?
- inner transition zone of prostate proliferates and narrows urethra
what are the symptoms of benign prostate hyperplasia?
- LUTS
- storage symptoms
- voiding symptoms
- if there is a totally occluded urethra- retention, hydronephrosis, UTI, stones
what are LUTS storage symptoms?
- frequency
- urgency
- nocturia
- incontinence
(FUNI)
what are voiding LUTS symptoms?
- poor stream
- dribbling
- incomplete emptying
- straining
- dysuria

W- WEAK STREAM
I-INTERMITTENCY/ HESITNANCY
S- STRAINING
E- EMPTYING INCOMPLETE
how is benign prostate hyperplasia diagnosed?
- DRE ( rectal exam )
- smooth and enlarged - PSA
- rule out prostate cancer - urine test
what is the treatment for benign prostate hyperplasia?
- lifestyle- decrease caffeine
- may need catheter acutely - drugs
- surgery - last resort
- TURP- transurethral resection of prostate
what is the first line of drugs used for BPH? what does it do?
Alpha blocker- tamsulosin
- relaxes bladder neck
what is the second line of drugs used for BPH? what does it do?
5 alpha reductase inhibitors - finasteride
- decrease testosterone production and therefore decrease prostate size
