May have difficulty with satisfactory resolution of the generativity VS stagnation. May develop dears of early aging or dying. Physical manifestations of the illness may reawaken the struggles and conflicts of previous stages.
How does chronic illness affect the older adult?
May be more likely to experience feelings of hopelessness (ego VS identity VS despair). The illness will likely influence the length of time the older adult can continue in an independent living environment or when to consider a nursing home placement
Emphasizing sustaining relationships
Ask patients to discuss significant relationships and encourage reminiscence
Attachment Ideation - preoccupation with a specific loved one
Reminding the patient that the loss of control is temporary
Let patients know in exacerbations of the illness that some of the equipment/procedures won’t be necessary when they go home and that you’ll be teaching them whatever will be necessary for self-care at home
Radiating Hope
The nurse should regard health problem as an opportunity for growth and adaptation. Practice unconditional positive regard by accepting the patient’s expression of emotions, even anger. Insisting that the patient maintain a positive attitude is not helpful, as it can trivalize the patient’s distress so this is to be avoided.
Expanding the patient’s coping repertoire
Support the coping mechanisms by the patient and/or teach or role model those that might be helpful such as:
- Seeking information
- Distraction
- Confronting the problem
- Actively participating with the planning and implementation of care
- Redefining the problem as something favorable
- Relaxation techniques
- Visual imagery
- Desensitization to anticipated stressful events
