Chronic Complications of Diabetes Flashcards
Damage to blood vessels secondary to chronic hyperglycemia
Angiopathy
Diseases of large and medium-sized blood vessels
i. Cerebrovascular disease
ii. Cardiovascular disease
iii. Peripheral vascular disease
Thickening of vessel membranes in capillaries and arterioles from chronic hyperglycemia
Microvascular Complications
Microvascular Complications; Areas most affected:
i. Eyes—retinopathy
ii. Kidneys—nephropathy
iii. Nerves—neuropathy
Microvascular damage to retina due to chronic hyperglycemia, nephropathy, and HTN
Retinopathy
Retinopathy: Care
a. Initially no changes in vision
b. Annual eye examinations with dilation to monitor
c. Maintain healthy blood glucose levels and manage hypertension
Risk factors for nephropathy
i. Hypertension
ii. Genetics
iii. Smoking
iv. Chronic hyperglycemia
Most common neuropathy
sensory
Most common form of neuropathy
Distal symmetric polyneuropathy “stocking-glove neuropathy”
Distal symmetric polyneuropathy treatment
i. Manage blood glucose levels
ii. Drug therapy
Autonomic Neuropathy can lead too
Hypoglycemic unawareness
bowel incontinence diarrhea
urinary retention
Skin complications
Diabetic dermopathy
Red-Down, round or oval patches
manifestation of insulin resistance
Acanthosis nigerians
Glycemic control challenges for older individuals
i. Increased hypoglycemic unawareness
ii. Functional limitations
iii. Coexisting medical problems
iv. Cognitive decline
Distal symmetric polyneuropathy s/s
Loss of sensation
Pain-burning, cramping, crushing, tearing; worse @ night
