Healthcare system
The medical and social services that provide healthcare support to Canadians, support that ranges from health promotion, to health maintenance, to long-term chronic care
Medical model
A healthcare model that favours surgery, drug therapy, and rehabilitation through physical therapies, with a focus on the treatment of diseases and injuries, usually in the physician’s office, a hospital, or other healthcare institutions
Social model
A healthcare model that includes personal and family counselling, home care, and adult daycare programs as part of the healthcare system, and that tries to keep older people in their own homes
Long-term care
Social and medical services, including formal services, home or institutional care, and family care, for people who have functional limitations
Health promotion model
A healthcare model that focuses on self-care and preventing diseases through lifestyle change, increased knowledge about healthy behaviours, and environmental improvements
Romanow Report
The 2002 Canadian federal government report of the Commission on the Future of Health Care in Canada, led by Roy Romanow; the report underscores Canada’s commitment to its healthcare system and makes several proposals for improving the delivery of healthcare to seniors.
Community Care Access Centres (CCAACs)
Ontario’s system of community care centres that provide a single point of entry into the healthcare system and services such as in-home care, physiotherapy, and dietetic services
Continuum of care
The array of services that range from institutional care to little or no institutional contact
Geriatric day hospital
A hospital program that offers a full range of hospital services to older people who live in the community and that assess individuals’ needs before setting up a healthcare plan
Adult daycare programs
Provide non-institutional support for people who cannot stay in their homes without it
Assisted living
Housing that provides personal care services and other supports to frail seniors to help them live independently in the community
Home care
A range of social and medical services designed to help people live independently in the community
Health field
The realm of health services, including the healthcare system, traditional medical services, improvements in human biology, improvements in lifestyle, and improvements in environment, as envisioned by former federal minister of health Marc Lalonde
Care gap
The difference between what care could or should be and what usual care is; the care gap leads to missed clinical benefits and higher costs for payers
Single-point-of-entry model
A single agency that assesses clients’ needs, coordinates service delivery from multiple sources, and monitors clients’ progress
