Fungal agents of importance
Cutaneous mycotic organisms- dermatophytes
Most commonly seen dermatophytes
M. canis, M. gypseum, and T. mentagrophytes
Anthropophilic
confined to humans
Zoophilic
parasites of animals
Geophilic
free-living saprophytes in soil
Most dermatophytes will grow on
the hair shaft and some can be visualized microscopically
Dermatophyte testing
Pluck hairs-periphery lesion, place on slide w/ 1-2 drops of 10% KOH, after 2-10 minutes small globular arthrospores attached to the shaft are visible
Contraindication with wood lamps
only 50% of cases show bc they must be at the right life stage
Dermatophyte test medium (DTM) turns _ in the presence of dermatophytes
red
Can be used in conjunction w/ DTM to accelerate formation of macroconidia used for ID and confirmation
Rapid sporulation medium (RSM) and Enhanced sporulation medium (ESM)
Sabouraud dextrose agar also promotes
early formation of macroconidia but no color indicator
Sample collection
clean lesion, collect samples from lesion periphery, incubate at room temp w/ loose cap
Test results
Red color in DTM,Teal in ESM or other media, must be supported by microscopic exam
Cultures of nondermatophytes are plated on
blood agar or Sabouraud dextrose agar
