Olfactory
(1) smell=have pt identify different aromas
Optic
(II) vision and visual fields=have pt read snellen-type
Oculomotor
(III) extraocular eye movement, movement of the ciliary muscles of the lens=assess pupil reaction and 6 ocular movements
Trochlear
(IV) extraocular eye movement, moves eyeballs down and lateral= assess pt’s 6 ocular movements
Trigeminal
(v) sensation of the cornea, skin of the face, and nasal mucosa=while pt looks upward, touch lateral sclera w/gauze to elicit blink reflex, have pt close eyes as you touch pt’s forehead and sinuses with cotton or blunt objects; have pt clench teeth
Abducens
(VI) extraocular eye movement, move eyes laterally=assess pt’s direction of gaze
Facial
(VII) facial expression and taste (anterior 2/3)=ask pt to smile, raise eyebrows, puff cheeks, have pt taste sweet, salty, sour
Auditory
(VIII) equilibrium/hearing=Romberg test and ask pt if they can hear the spoken word or tuning fork
Glossopharyngeal
(IX) swallowing ability, tongue movement, taste (posterior)=apply taste to posterior part of tongue, ask pt to move tongue up and down and side to side
Vagus
(X) sensation of the pharynx and larynx, swallowing, vocal cord movement=have pt move tongue around and assess speech for hoarseness
Accessory
(XI) head movement, shrugging of shoulders=ask pt to shrug shoulders and turn head from side to side
Hypoglossal
(XII) protrusion of tongue, move tongue up and down and side to side=ask pt to protrude tongue at midline and move side to side
