A

Right question: Why did what they were doing, at the moment in time when things went wrong, makes sense to them? This helps you step into the shoes of the responders and perhaps their mindset to understand how they saw and interpreted the situation.

The question to avoid is how could they be so stupid.

  2. What was the overall mission of the incident, and what role were the responders playing in advancing the mission when things went wrong?
  3. Why did it make sense for the responders to be doing what they were doing at the moment things went wrong?
  4. What task or objective or responders focused on that kept them from seeing the bad outcome on the horizon?
  5. Whether any distractions or interruptions that do the responders attention away from their task?
  6. Did the responders understand the overall incident goals and objectives their roles in the mission, and how other individuals/teams were going to influence the outcome?

A

Asking the right questions allows him to infer about the events that were unfolding around them. Which helps learn lessons from a different set of my the mindset of the student.
