Ch 25 resp Flashcards
difference w adult
- shorter neck
- trach increase in length until 5yr
- bronchi split higher- especially rt
- flexible cartilage-need proper pos
lower resp
- alveoli dev at 32wks- no gas exchange before 24wk
- bronchioli narrow w less alveoli (8yr)
- diaphragm used for breathing until 5yr
- higher O2 consumption
foreign body aspiration
- chew and swallow not until 4yr
- right lung
- choke, dyspnea, tachypnea
- sit forward w neck extended
- asymptomatic after cough 15-30 min
- chronic cough, recurrent pneumonia, lung abcess-no response to meds
RR newborn, 1yr, 3yr

- 16-20
- 12-20
hypoxia S/S
increse restlessness, irritability, unexplained sudden confusion, rapid heart/RR
ARDS
- hypoventilation of lungs
- does not respond to O2
- sepsis, pneumonia, meconium aspiration, stomach contents aspiration, smoke, drown
apnea
- no breath 20+sec
- decreasing RR critical sign
central apnea
complete cessation of breath
obstructive apnea
absence of nasal airflow when respiratory efforts present
-sleep 3-6yrs-> tonsils and adenoids hypertrophy
mixed apnea
central resp pause that either preceedes or follows airway obstruction
apnea treat
caffiene, positioning, tactile stim
-toxic- tachyC, tachyP, jitteriness, tremors, seizure, vomit
apparent life threat event
apnea w skin color change, limp muscle tone, choke/gag
SIDS
- sudden death under 1yr during sleep
- 2-4mo
- frothy blood tinged sputum
- pacifyier helps
SIDS risk
- preterm/LBW
- native american, black
- males
- maternal smoke, drink, drugs
- SES
initial resp fail s/s
- restlessness
2 TachyP - TachyC
- diaphoresis
early decompensation S/S
- nasal flare
- retractions
- grunting
- wheezing
- anxiety/irritable
- mood changes
- headache
- HTN
- confusion
severe hypoxia and imminent resp distress s/s
- dyspnea
- bradyC
- cyanosis
- stupor and coma
croup
- no visual inspection or throat culture
- abrupt nighttime onset
- afebrile
- resolves 24-48hrs, maintain airway, oral hydration
epiglottitis s/s
- dysphonia-talking hurts
- dysphagia-swallow
- drooling
- distressed resp w stridor
- can’t lay flat-tripod
epiglotitis tx
- no inspection/culture
- intubate
- acute drooling ominus
- don’t cry
bacterial tracheitis
- secondary infection after laryngotracheobronchitis
- lies flat
- worse after nebulizer
