Ambulatory
Minimal systemic symptoms
Mild-moderate 1st line
Sulfasalazine
-When? colonic
Budesonide CR
-When? Ileocecal, right-sided disease
Moderate-severe active CD
Failed therapy for mild-moderate
Systemic symptoms
Moderate-severe 1st line
Systemic corticosteroids or IV corticosteroids–> if hospitalized
+
Biologics +/- AZA/6-MP/MTX
-When? patient failing steroid therapy, steroid-dependent
Other: methotrexate
Severe-fulminant active CD
Persistent systemic symptoms
Severe-fulminant 1st line
IV corticosteroids x 3-7 days
THEN
Infliximab
Maintenance CD
AZA/6-MP: patients in steroid-induced or infliximab-induced remission
Methotrexate: patients who induced by MTX
TNF-inhibitor: infliximab/adalimumab + AZA/6-MP
