What is cardiogenic shock
Decreased myocardial contraction force causes decreased ejection fraction and described cardiac output
What causes cardiogenic shocks
- Ischemic heart disease and AMI
- Involves left main or LAD obstruction
- Requires at least 40% loss of functional myocardium
What are other causes of cardiogenic shock
- Dysrhythmias
- Cardiomyopathy
- Valvular disorder
*associated with a high mortality rate
*not CHF
What is on the PE for cardiogenic shock
- Hypotension
- Narrow pulse pressure
- Tachycardia
- Tachypnea
- Cool, clammy skin
- Peripheral cyanosis
- Gallop rhythm
- JVP distention
- Pulmonary rales
What are findings on CXR during cardiogenic shock
- Cardiomegaly
*signs of pulmonary venous congestion
What are common findings on ECG during cardiogenic shock
- Non-specific ST segment and T waves changes
- Dysrhythmias
- Conduction abnl
What are common findings no echo during cardiogenic shocks
- Enlarged heart with Hypokinetic LV and decreased EF
What is found on the Cath during cardiogenic shock
LAD
*left main occlusion
*poor LV function
*reduced EF
What is the treatment of cardiogenic shock
- Continuous monitoring
- High-flow oxygen or mechanical ventilation
- Reversal of hypotension: vasopressors
- Inotropic support
*drugs to increase myocardial contractility and CO

*can do gentle fluid challenge 250CC if no overt pulmonary edema
What type of into-tropic drugs are given in cardiogenic shock
- Dobutamine (positive inotrope)
- Epi (positive inotrope and vasoconstrictor)
