What type of hypersensitive reaction is anaphylactic shock
IgE mediated
*airway risk (angioedema, tongue, laryngeal edema)
*need early intubation
What is the dosage of epi given to children and adults
Children
*0.01mg/kg IM mid outer thigh
Adults
*0.3-0.5mg IM mid outer thigh
*no absolute CI to epi in the setting of anaphylaxis
What is given if refractory to IM epi
Iv infusion
1. 0.1mcg/kg/min
*titrate the dose contrinously according to blood pressure, cardiac rate and function and oxygenation
What is given during anaphylactic shock
- IV fluid adminsitration-crystalloid (bolus)
- Bronchodilator-albuterol by nebulizer
- Histamine Blockade
- Glucocorticoid
What histamine block is given for anaphylactic shock
H1
*diphenhydramine
H2
*pepcid
What type of glucocorticoid is given during anaphylactic shock
Methylprednisolone
If a patient is on a beta blocker what are they given
Glucagon
What are the signs of anaphylactic shock
- Pruritus
- Hives
- Angioedma
- Distress
- Lump in throat
