How do thiazides work?
Inhibit reabsorbtion of K in the distal tubule!
List the three types of diuretics.
Thiazides, loop, potassium sparing.
Thiazides good for? Bad for?
Good HTN , heart failure, DI
AVOID in gout!
Where do loop diuretics work?
Ascending loop of henle.
Who gets loop diuretics?
Severe HTN, CHF, never alone. lasix is a loop.
Side effect of loop diuretic?
Ototoxicity.
What is potassium sparing diuretics used for?
CHF/ post MI. Don not use with ACE/ARBS. Causes gynecomastia.
What is midamor?
Potassium sparing diuretic.
What is the drug of choice in HTN with DM?
Ace inhibitor. Pril!
Who are ACEI contraindicated and the most common side effect?
Prege, cough.
What are the alpha blockers?
Zosins.
Side effect of alpha blockers?
First dose hypotension or syncope. Dry mouth dizziness.
What’s are the non cardiac use of alpha blockers (zosins)?
BPH, pheochromocytoma give alpha before beta blocker!
Who do CCB work best on?
Blacks and elderly
Most favorable non dihydropyridine?
Diltiazem, good for a fib and a flutter
