Digoxin (Lanoxin)

A

pharm class: cardiac glycoside
competes for a site on the pump that potassium binds to resulting in a increase in Ca2+ in heart cells
positive inotrope
increases BP
increases cardiac output
slows AV node
HOLD is HR is less than 60 BPM
Ideal plasma levels is 0.5-0.8 ng/mL
Toxicity is above 2.0 ng/mL
purkinge fibers create dangerous dysrythmias

Nitroglycerin

A

pharm class: antianginal drugs
vasodilation and causes blood pools in veins, less returns to heart = preload
sublingual dissolves under tongue
dose is 0.3 - 0.6 repeat every 5 minutes x 3 then call 911
use prophylactically
need a 10 hour break from patches
store in a cool dark place (Nitrostat)
